Location: Jones Campus 3518 West 25, Cleveland, OH 44109
32 - 40 hours per week; to include at least one weekend per month.
Responsible for completing admission assessments, patient histories and conducting physical examinations in accordance with physician orders; Coordinates, monitors and evaluates assigned clinical, psychiatric and other nursing assessments; Serves as a member of inter-disciplinary team to develop patient care plans and case management plans; Provides/coordinates effective case management services accordance to payer source guidelines by providing/coordinating timely referrals and proper guidance and instruction to applicable personnel; Prepares health clinic procedures where indicated in compliance with Ohio Department of Mental Health and Joint Commission; Provides oversight of LPNs; Prepares health clinic nursing schedule by month; Administers medications as prescribed by physician.
Requires a Bachelor's degree in Nursing (preferred concentration in mental health) with valid RN license in the State of Ohio.
Must possess a valid Ohio driver's license with less than 6 points, personal transportation and proof of auto insurance.
